To Register
To register for the Pro-Vision Academy Holiday course or courses of your choice you must complete this online form. Once you submit your entry you will be able to choose the type of money saving pass you require and pay your registration online.
If you don't want to pay online simply go to the online registration form print it and bring to the clinic completed together with payment.
Costs
Daily walk in fee: £15.00
Year Pass 2009/2010: £450.00 (Summer Break 09, Autumn Half Term 09, Spring Half Term 010, Easter Break 010, summer Half Term 010, , includes 55 sessions, saving £375.00)
Summer Break Pass: £250.00 (includes 30 sessions, saving £200.00)
Week Pass: £50 (5 sessions, saving £25.00)
When: Summer Break July-August, Autumn Half Term Oct. Spring Half Term Feb, Easter Break Apr, Summer Half Term May,
Where: Close Park Dumers Lane Radcliffe (subject to change dependent on weather)start 09.00AM-15.00PM please bring a packed lunch.
Who: Open: youth players, any skill level, both boys and girls, from 6 to 16
To enter multiple players: You can complete and submit this entry form for two players. After you submit the online form you will be able to pay for each player.
